How does it know what is a heading?+

By size. It takes the median font size on the page as the body text, then treats anything meaningfully larger as a heading - the bigger the jump, the higher the level. It is a good guess rather than a certainty, so check the result on an unusual layout.

Does it keep images and tables?+

No. It converts text structure only. Images are not extracted, and a table comes through as its cell text rather than as a Markdown table.

Will it work on a scan?+

No - a scan has no text layer. Use the OCR tool first, then convert its output.
